covered benefits; the member is responsible for the full cost of all
goods and services purchased. Basix, LLC develops and operates the
pehpPLUS program through a contractual relationship with PEHP. PEHP,
Basix and other entities involved with the pehpPLUS program do not
endorse, and have no responsibility and/or liability for any aspect of
the services or goods offered through the pehpPLUS program. Please be
aware that complications from non-covered services, for example cosmetic
surgery, are not covered benefits. The goods and services available
through the pehpPLUS program as well as their pricing and other terms
may change without notice. Please confirm that the pricing and other
terms are still in effect when you call the vendor. Be an informed
consumer. Be sure to consult your physician prior to starting any
exercise program, choosing a surgical or medical procedure and before
making dietary and lifestyle changes, including the use of vitamins and
supplements.
Participants in the PehpPLUS program acknowledge that there
is a possibility that contracted vendors providing goods and/or services
in connection with the Program can cause injury and damage to a
participant. The participant understands and accepts such risk. PEHP
and its program contractor, Basix, disclaims any warranty, expressed or
implied, regarding the quality, nature or kind of goods and/or services
to be rendered to a participant. PEHP and/or Basix shall not be
responsible or liable to the Participant under any circumstances for any
injury or damage sustained by the Participant as a result of goods
and/or services provided under the Program. PEHP and/or Basix shall not
be liable for any damages, including without limitation, direct,
indirect, consequential, incidental, special or exemplary damages
arising out of or related to the provision of goods and/or services to
the Participant.
